Exact Artwork Sizing

Have watched the videos on sizing, but still left with questions.

is listing his posters 16x20” - 40x50cm (example), but what size do I design the actual artwork for printing? 16x20” converted would be exactly 40.6 x 50.8 cm. And 40x50cm converted is 15.75 x 16.69”. (Same ratio, I understand that)…

What size will actually be printed? Are picture frame sizes different in US to UK? I don’t want to create problems when selling to different countries and risk that the print won’t fit the frame?

Are printers printing to exact measurements the artwork is in or do they make them fit to the standard framing size in their location (i.e. in US to fit 16x20” frame, and UK to fit 40x50cm frame)?

Anyone had any experience or customer feedback/problems? Thanks
