Rejected Ad

Hi Sally, I have a current Meta campaign running and it has been working fine for quite a few days. All of a sudden I received a message saying my ad is rejected as it violates advertising standards. Meta says it is because I am collecting data from a leads ad, but I have always done this. I have attached the message I received. Can you please let me know what I have done wrong and how to fix it? Thank you
