Heathcote -------- Q: What was it like before the fellowship, and could you summarize the situation you were in a year ago? Traian Stroia: I mostly lacked confidence. I had almost no feedback regarding my reports. It was difficult because I am a young consultant radiologist, so I did not have so much experience. It is very hard to find someone who can help you with your MSK cases... Before, a knee was about 30 minutes. Now it is no more than five. Q: Could you introduce yourself and say what you were doing professionally a year ago? Traian Stroia: I am not an MSK-only based radiologist. I do a lot of CT and oncology and I am now even faster on all my reports and more confident. I work in a psychiatry hospital and also mostly in private practice. Only in private practice, MSK. Q: What were the main problems you were having before with MSK cases? Traian Stroia: The lack of feedback, that was the biggest issue. I did not know if my reports were good... And always having second thoughts. That report, was that a tear or not? Did I write that thing correctly? Q: What has been the impact on your career? Traian Stroia: I feel better. I am faster. I earn more money... Before, I had maybe 10 cases per day, CT and MRI, and now I am over 30 in the same amount of time. I have more time with my family and I am overall more satisfied with how my work is in general. Since doing the fellowship, my earnings have increased three or four times. Q: How would you describe the fellowship process?

He further described the tear as a "flipped meniscus" with a large intercondylar fragment, emphasizing that it was a special form of a bucket handle tear, and advised against using the term "hemi bucket" (00:10:23). - Iman's Hand Case: Rheumatoid Arthritis Iman presented a case of chronic pain in a hand, leading to a discussion on rheumatoid arthritis (00:06:25). Christoph Agten identified synovitis in multiple compartments and tenosynovitis as strong indicators of inflammatory arthropathy, recommending a rheumatological review and lab tests despite the absence of erosions (00:07:45). - Anisha's Knee Case: Radial Tear of Medial Meniscus Anisha presented a second knee case involving a radial tear of the medial meniscus with extrusion and degeneration (00:13:00). Christoph Agten described it as a large radial tear, noting the truncation of the meniscus and substance hanging into the meniscotibial gutter, explaining that the loss of ring stress caused the meniscus to squeeze out (00:15:00). He also discussed bony irregularities and bone marrow edema in the medial tibiofemoral area, suggesting chronic friction and advising descriptive reporting rather than specific grading (00:17:49). - Iman's Foot Case: Chronic Pain and Isolated Lesion Iman's second case involved chronic pain in a 73-year-old male with an isolated lesion in the lateral cuneiform bone, prompting a query about the lesion's isolated nature (00:21:17). Christoph Agten attributed the finding to a subchondral insufficiency fracture or stress reaction, potentially exacerbated by compensatory overuse, and noted associated tendinopathy in the peroneal tendons (00:22:41). - Anisha's Shoulder Case: Post-Labral Repair Anisha's final case involved a shoulder post-labral repair, with a question about a SLAP lesion type (00:26:10). Christoph Agten initially observed fraying and degeneration of the superior labrum but noted that an arthrogram revealed widening of the recess and contrast within the labrum, leading him to upgrade the diagnosis from a SLAP 1 to a SLAP 2 tear (00:28:35). He also discussed the appearance of the MGHL and SGHL ligaments, noting potential injection-related findings and variations in ligament nomenclature (00:33:56). - Meeting Conclusion and Tool Promotion Christoph Agten concluded the Q&A session by promoting "finalscribe," his voice recognition tool, offering a seven-day free trial (00:37:16).
