Athletic pubalgia / Sports hernia
Is there currently any role for dynamic MRI scans to assess the weakness or excessive bulging of the posterior wall of the inguinal canal in patients with “sports hernia”. Should a dynamic protocol be included in the imaging assessment? Inguinal hernias will mostly be diagnosed on non-dynamic scans. In general the current imaging protocols focus around the pre-pubic aponeurotic complex injuries (rectus - adductor longus aponeurosis) and other causes of hip and groin pain.

I didn't base my practice on this one article, we have been doing these fast T1 scan with straining for years. That is just an example article I found in about 5mins - feel free to do a proper lit review. 3D lava - from my experience, that's a long sequence and nobody can valsalva that long without passing out or stopping mid scan. This is a quick smash and grab sequence to satisfy the desire of the referring clinician - is this ortho issue or gen surgery issue. The report always contains a caveat to consider dynamic US for definitive assessment but at that stage, you normally have ruled out major MSK causes with the MRI.
