@Ahmed Hadi that is a valid thought but the patient is 26 year old. Insufficiency fractures are more common in the elderly patients.

@Christoph Agten thanks. the patient had a prior surgery outside with a pathology report of Teno synovial giant cell tumour. The tumour appears to have come back more aggressively.

@Christoph Agten Do you think there are two separate lesion ( the larger one in the posterior aspect of the ankle extending into the sinus tarsi and a smaller one surrounding the tibialis posterior tendon) or are they communicating.

@Daniel April thank you for the feedback, the patient had a prior injury as well and the bony proliferations were picked up on an X-ray done a few months ago. what are your thoughts on the status of the peroneal retinaculum. patient has a subjective complaint of the sensation of the tendon dislodgment. He is set to come back for a follow up ultrasound.
