Which sentence in business should immediately make a founder slightly nervous?

My nominations:

“I thought someone else was doing it.”

“That’s not how I was told to do it.”

“We normally just ask you.”

“Nobody said it was urgent.”

“The customer said you promised…”

“We’ve always done it that way.”

All innocent enough.

But each one can be a tiny flare going up over a bigger problem:

ownership, communication, standards, decision rights, expectations or alignment.

So add your own.

What phrase have you heard in your business that made you think:

“Ah. That sentence just explained considerably more than the person saying it realised.”

Best one earns absolutely nothing except the satisfaction of making the rest of HQ wince in recognition.
