But my English is not that good...
If you are thinking why I chose to keep all information in the community in ENGLISH let me explain: A lot of you may not consider yourselves "fluent" or "good enough" to talk and write in English so this is your 1st step working on that. You are probably better than you think but practice and repetition are the way to grow and get better. I recommend you interact with other members in English as much as possible as a way to get used to it. I will discuss in the future other methods of implementing this and increasing your vocabulary. You are capable and worth more than you give yourself value so keep your head up and let me help you with whatever you need.

S - Patient reports sudden onset of shoulder pain, with symptoms more frequent at night, especially when trying to find a sleeping position. Patient reports pain during sleep and that movements tend to aggravate symptoms. O - Incomplete range of multidirectional movement is observed, both active and passive, with external rotation movement being more compromised, pain scale 7/10, preserved muscle strength and deficit of internal glenohumeral rotation. A- According to reports and clinical evaluation, the condition is suggestive of frozen shoulder. P- Joint mobilization exercises, muscle and capsule stretching are recommended 3 times a week with guidance on home activities.

SOAP S Patient reports pain in the plantar region of the right foot when getting out of bed in the morning, rated as 8/10 on the pain scale. Pain decreases throughout the day when walking, but increases when sitting for a long time and when standing up to step the pain returns. There is no previous history of foot injuries. He reports that he has started running. O Swelling around the right heel is observed, tenderness upom palpation in the plantar region from the heel to the toes. Muscle strength tests indicate 4/5 of the plantar flexors and dorsiflexors. Range of motion is not affected and without change during gait assessment. A The findings are consistent with plantar fasciitis, probably due to the onset of running and inappropriate use of sneakers. P Provide guidance on proper use of running shoes, ice application and initiate a program to strengthen the plantarflexor and dorsiflexor muscles, stretch the plantar fascia and release myofascially. Recommend follow-up in two weeks to assess progress
