SOAP Notes - Homework
Good morning! Sorry my delay on last week homework! 🫣 S – A 75-year-old male presented to the clinic experiencing lower back pain the past 15 days, which worsens when standing. He also related weakness in both legs. O – Pain level is rated at 5 out of 10, with limited forward flexion and lateral bending. Strength 4/5 in lower extremities. Gait with difficulty. Positive for straight leg raise test on the right side. A – Pain and discomfort in the lower back, decreased ROM, muscle weakness and impaired gait. Diagnosis: Lumbar strain with associated muscle spasm. P – Improve lumbar flexibility and ROM. Strengthen lower back and legs muscles. Normalize gait.

Very good, Viviane! Not much to add to your SOAP note but I'll discuss simple things to make it more complete. S- Perfect. You have the patient basic information (sex, age, reason for looking for a PT) O - I would just add where the limited forward flexion and lateral bending is. I know it is in the lumbar spine but it is always good to make it clear. A - PTs can't make a medical diagnosis so even though you are correct in the diagnosis is important to write: all signs and symptoms are clinically associated with lumbar strain due to lumbar muscles spasm. P - Perfect. Nothing to add

S- The patient comes to the appointment and complains of pain and tingling in the thenar area of the right hand. The patient works full-time at the bank. She lives by herself in an apartment on the 2nd floor. O- upon evaluation, the Patient presents with waist muscles over the thenar area. A- wrist flexion strength 3+/5, Phalen's test +. P- mobilization of the medial nerves. Analgesics strategy for pain Exercise to strengthen the wrist flexor.

Very good! I'll just address a couple points: S - All information in here but if you can make it a single paragraph is preferable. Imagine you are telling a history to someone. O - Muscle testing and Phalen Test of the Wrist should be here. It is good to repeat that it was the R hand/wrist. A - Use the information of the Objective part to make your Assessment. For example: Based on decreased strength of R wrist flexion and extension, swelling in thenar area and positive Phalen test, clinical condition is associated wth tendonitis of the wrist flexors/synovitis of the carpal joints/etc. P - Perfect. YOu recommended analgesic strategies and not analgesic medication. That is correct for a Physical Therapist! Very good!!

S – Patient reports a dull, constant ache in the left knee, rated at 5/10 on the pain scale. Pain when getting up or sitting down from a chair and climbing stairs. Symptoms worsen throughout the day and improve with sitting. O – Swelling observed around the right knee, stiffness upon awakening and range of motion for the knee is limited to 90 degrees of flexion. A – Likely osteoarthritis due to advanced age, work activities with repetitive movements and joint overload. P – Prescribe rest, recommend anti-inflammatory treatment quad-strengthening and analgesics strategy. Exercises twice a week and suggest a follow-up in one month.

Very good! I'll just address a couple things: S - You can add the pain scale here or at the Objective section. O - I know you said in the Subjective part that it was the Left knee but it is important to repeat here. A - Perfect! P - You can recommend anti-inflammatory strategies (rest, gentle mobilization, ice/hot packs) but Physical Therapists can not recommend medication. Great work!
HOMEWORK
S | Patient reports shoulder pain, a long time, rated at 6/10 pain scale. The pain increases as the effort is made, in a way that hinders work and daily activities. O | Decreased shoulder mobility, especially for movements above 80 degrees. Disconfort during palpation of the anterior shoulder region and decreased strength for shlouder abduction and extension. A| Results shower for inflamation process in muscle supraspinatus. P | Recommended passive mobility training for the glenohumeral joint, stretching and strengthening of the supraspinatus muscles

Very good! I'll just correct a couple things: S | It's ok to add the pain scale here but you can add it in the objective section as well. O | Better if you describe for which shoulder movement is painful above 80 deg (flexion, abduction, extension, external rotation, internal rotation). Also add the strength grade since you tested for it. A| Show instead of shower. Other than that, perfect. P | Perfect
