Position execution

For some reason some of the positions keep getting out of sync. For example when NQ get into a position sometimes it misses exit or just doesn't exit and when i open the server it asks me if i want to exit or keep the position. And i have to keep manually match the true/false positions. Does this happen often or is there something wrong? i have in my settings that if a limit order doesn't get filled in 15 seconds it switches to market order.
