Most teams wait for AI to fail in production before they analyze it. That is not governance. That is postmortem management. In AI transformation, failure modes should be mapped before deployment: where the model will be overconfident, where data will drift, where humans will overtrust outputs, and where incentives will quietly distort usage. Technical risk is only one layer. Behavioral and organizational risks are usually larger. If you cannot clearly describe how your system will fail, you are not ready to scale it. Mature AI strategy is not about preventing all failure. It is about designing how failure is detected, contained, and learned from before it becomes systemic.
