One Time Fee or Monthly retainer?

Hey everyone I was just curious about what should I do one time fee or monthly retainer.

I was planning to do $(x)k Setup fee and and 3% commision per deal.

For 3 months.

But I'm confused if my client wants to extend the contract to 6 months can I charge him/her the setup fee again or what?

And please explain how these things work I really don't have idea about this.

I'm charging monthly retainer but I think I can do better with One time setup fee (but I don't know how that works)
