Mystery Diagnosis/Diagnoses -What is your Dx?
Take a stab at it, leave what you think was causing all this in the comments below.... 7 years ago patient had neck adjustment, L fascial numbness, L arm numbness, L genital and glut numbness and L LE weakness. Neurologist x 2 scanned MRIs were negative, MRA of brain negative, EMG negative, PTs, chiros etc etc no help since then I had her strength back and numbness gone in 5 mins! Hint: There were 3 diagnoses

TMJ - What Causes it, What to do....
One of the reasons I see so much failed TMJ treatments in my clinic is because their actual cause of it was missed. They get night splints, scans, injections etc but no lasting relief. Years ago, I started to ask myself why are these muscles locked down around the TMJ. I had an insight, they were stressed! Clinching was subconscious to them and a lot were grinding in their sleep. I also saw a pattern, that these patients also had neck hypertonicity, headaches a lot of the time in addition to TMJ. In these patients they had too much stress. I call these "Stress injuries". When I identified that root cause for them was stress I would tell them that and they would argue with me saying I am not stressed! Most people live at constant level of stress that they are unaware of I found. I asked them to meditate but patient after patient refused so I gave up on that as I realized I had better luck asking them to change their religion! (literally) A great help was a GOOD CBD 3x a day at a dose they feel relaxed at in 2.5 hrs. Side note: I have found TMJ patients to be the least complaint patients I have ever worked with! What has worked for you?

@Andreas Roeim you know when I posted that I fully intended to say AVN and disc issues are not able to be helped by me. Then I typed it and posted it and forgot! You're right, I always ask what did the dentist say disc or AVN? if not I treat for stress!
Back Spasms & Triggers & Treatments
I have mainly a chronic low back pain clinic (SI Jt/Sacral Torsion Shear). In any given time of year (except the holiday season) 10-20% of my patients go into a a full back spasm. When I check these patients they have acute SI Jt/sacral torsion and shear. The severe ones take 3 days to get out of bed and 1-3 weeks to calm down. I check if they have bowel and bladder dysfunction and myotome weakness and make sure there was no trauma. If none present, it is back spasm. These need to calm down you cannot exercise your way through it I find it makes my patients worse. I tell them let it calm down stop everything and slowly re-enter their exercises when you are SURE you are back to baseline. Over the years I have found these to be the triggers of that neuro protective state: - Emotional Stress! (major one), general stress increases or even severe deaths/grief process which can be delayed. - Vacuuming, sweeping mopping - Long car rides - Plane flights How you manage your patients psychological state when these happen dictate if they quit and stop the rehab or let it calm down and finish the rehab and get better. It is an art!

Stroke client question
Hi everyone. I am new to this group and have just started applying what I haven't learned with the myofascial referral method and have had much success. I do have a question, I have a post stroke client (Aug 2026) who still has tightness in his effected knee, I have done foam rolling via stick and stretching but he says its still like a turnic around his knee. I am guessing its something nerve related, but not sure. Is there something I can do using the referral method to help?

that can be complicated due to the stroke, however, I would use my SI eval method to screen for referred pain into the knee from the SI. If it relieves when you realign and hold the SI open then it indicates SI referred pain into the knee. Im glad you having success with the myofascial method!
