💪 TESTOSTERONE CYPIONATE: WHAT IS IT?
If you've heard someone say they're "on TRT" in the U.S., there's a good chance they mean testosterone cypionate. Here's what it is in plain English.
WHAT IT IS
Testosterone cypionate is testosterone with a "cypionate" tail (called an ester) attached. The ester makes it release slowly from the injection site instead of all at once. It comes dissolved in oil and is an FDA-approved prescription drug for men with confirmed low testosterone. In the U.S. it's a controlled substance.
WHAT PEOPLE USE IT FOR
• Treating diagnosed low testosterone (hypogonadism)
• Improving low sex drive, low energy, and other symptoms linked to confirmed low T
• It's the most common injectable testosterone in the U.S.
WHAT THE RESEARCH ACTUALLY SHOWS
• In men with truly low testosterone, large studies show improvements in sex drive and sexual function, plus smaller benefits for mood, bone density, and anemia.
• A large 2023 trial (TRAVERSE) found testosterone therapy did not increase major heart events compared with placebo in middle-aged and older men with low T and heart risk. It did show more cases of some other problems, like an irregular heart rhythm called atrial fibrillation.
• For men with normal testosterone, the benefits are much less clear.
Evidence level: Strong for treating confirmed low T.
COMMON QUESTIONS
Cypionate vs enanthate? They're very similar. Cypionate lasts slightly longer, and which one you see is mostly about what's available in your country.
How often is it injected? The official label allows injections every 2 to 4 weeks. Many clinicians now spread it out into smaller, more frequent injections to avoid big highs and lows. The schedule is a decision for the prescriber.
Does it affect fertility? Yes. Outside testosterone tells the brain to lower LH and FSH, which can greatly reduce sperm production. That's why fertility plans come up before starting.
What gets monitored? Usually testosterone levels, hematocrit (red blood cell level), and PSA for prostate health, along with symptoms.
