Building quoting system

Can anyone help me gather some info on how to build a price quote forum, similar to the one in the picture. (see picture below) I know it's not much to go off of but basically it asks questions about the equipment the customer has and will quote them after amswering all fields.

I'm not looking for someone to build one for me, rather I would love to learn how to do these so I can continue to build in any field that helps me.

Any info or even a point in the right direction would be super appreciated.
