Hi everyone! Iโ€™m Natasha Jackson, RN, CEO of iCareRN, and Iโ€™m so excited to welcome you to our community. ๐Ÿ˜ Let me share a bit about myself and how I got here. Iโ€™ve been a nurse for over 20 years, and about two and a half years ago, I took a leap of faith and started my own care management business. At the time, I had no formal experience in care management, just my clinical expertise, a passion for helping patients, and a drive to make a difference. Fast forward to today, Iโ€™ve built my business from the ground up, signed my first six-figure contract this year, and learned so much along the way. It wasnโ€™t easy, and I certainly didnโ€™t do it alone. With the guidance of coaches, valuable resources, and a lot of hard work, I gained the confidence to turn my vision into reality. Thatโ€™s proof to me that with the right mindset, mentorship, and resources, anything is possible. Now, my mission is to share what Iโ€™ve learned to help other nurses avoid the pitfalls I faced and build successful, sustainable care management businesses of their own. This community is a space for us to grow together, share ideas, and support each other in this journey toward entrepreneurship. Weekly Update
Hey everyone. I want to start by saying thank you for being here and for your patience with me this week. I wonโ€™t be live for our weekly session tonight, instead, Iโ€™m sharing these slides to make sure you still get value and clarity around CCM. Today, weโ€™re going to focus on Chronic Care Management. What it is, who itโ€™s for, whatโ€™s changed, and how it fits into care management practice today. The information Iโ€™m sharing comes directly from the Medicare Learning Network, the MLN document, updated June 2025. This is the most current guidance CMS has released on Chronic Care Management, and itโ€™s meant to be a practical resource you can use to understand how CCM works in real practice. As you review, I want you to think about how CCM shows up in the work nurses already do, and how understanding this service better can help you position yourself more intentionally going forward. If you have questions, please drop them inside the Skool. I will make sure to review them and respond. Weโ€™ll be back live next Wednesday at 7 p.m. Eastern. During that session, weโ€™ll talk more about independent care management, how nurses can position themselves, different ways to work beyond traditional roles, and where CMS is heading.
