Hey everyone 💙 I know I haven’t been as active inside the community lately, and I wanted to check in and say thank you to everyone who has continued to stay connected here. Over the past few months, I’ve been spending a lot of time refining my direction professionally and building out the consulting side of my work in care management and value-based care. As I continue growing in this space, I’ve also been thinking deeply about how I want to better support nurses, healthcare organizations, and the future of nurse-led care management. This next season will be more intentional. Over the coming weeks, you can expect:• more educational content• more discussions around care management and value-based care• more insight into healthcare operations and workflow strategy• more engagement inside the community• and more real conversations about the evolving role of nurses in care management. Hi everyone. I’m a FAmily Nurse practitioner for over 20 years. I currently works in Primary care clinic. I stumbled on Mrs Natasha Jackson’s video on YouTube about Chronic Care Management which is something I have been thinking of for the past three years. I didn’t know there was such a program (CCM)paid by Medicare and most private insurance companies. I am so interesting in learning more about how to start my own business as a Chronic Care Management. So I’m glad to join this community.

Hi everyone, I wanted to share a quick update with you. For now, I’ll be pausing our weekly Wednesday meetings as I take some time to restructure how I’m delivering content and support inside the community. This isn’t me stepping away it’s me being intentional about how I show up and making sure what I’m building here actually serves you in a meaningful way while still giving me space to grow and focus on my own business. You’ll still see me inside the community. I’ll be sharing more educational content, videos, and insights around care management, value-based care, and how to build your own business in this space. If you have questions, I want you to post them here. That’s what this space is for. I’ll be checking in and responding. Live sessions will return with a more structured format once I’ve aligned things with my schedule and the direction I’m building toward. I’ll keep you updated on timing and what that will look like.
