Follow-up on your post with RPM codes
I am new and just got my first patient from a group home. I saw him last month on 2/19/26. So I want to use him as example to make sure I am understanding well. So these are the codes that I should use to bill A) Setup / patient education - 99453 – Initial setup + patient education on use of equipment B) Device supply + data transmission - do I wait for 30 days to be over, so that will be this month on 3/20/26 that it will be 30 days. So I check how many readings I have and that will determine which code I use? - 99445 – RPM device supply with daily recordings/alerts and transmission for 2–15 days in a 30-day period. - 99454 – RPM device supply with daily recordings/alerts and transmission for 16–30 days in a 30-day period. C) Treatment management (time + interactive communication) - 99457 – RPM treatment management, first 20 minutes in the calendar month requiring interactive communication. Just looking at the month of February or do I go from 2/19/26-3/20/26 - 99458 – Each additional 20 minutes (add-on to 99457) Is it better to keep things simple and just do from 3/1/26-3/31/26? Also, when you establish care, what initial visit documentation do you do? over the phone? what codes do you use? I am an NP and right now I have not approached practices and trying to get comfortable and just approaching group homeowners. I am planning on going to elderly communities as well and events as well. Do I send a record of the patient BP to their PCP? Thank you!

Great Question
Remote Patient Monitoring 101
Tonight at 7:00 PM ET | Empower Care Academy Live Join me tonight for a 30-minute live conversation on Remote Patient Monitoring (RPM). This is RPM 101. Practical, real-world, and straight to the point. We’ll cover: - What RPM is and how it actually works in primary care - What can be billed and what has changed recently - How RPM fits alongside care management - What implementation really looks like - Common questions and misconceptions - This is not a sales pitch and not a deep technical training. It’s about understanding the model, the clinical role, and the opportunity for nurses inside CMS-aligned care. I’ll also be sharing a RPM 101 PDF to support the discussion and give you something concrete to reference after the session. If you’re a nurse curious about RPM, care management, or how these programs work together in real practice, this session is for you. Come live if you can. If not, a replay will be posted. See you tonight at 7 PM ET.
