Here's How I Made $123,000 in my Clinic in May Working 20+ Hrs a Week!
For years, I would tell people that I was getting exceptional results with patients who had already failed just about everything else. Most people didn't believe me. So I published my diagnostic procedures and treatment protocols for multiple conditions. Those papers went on to achieve readership numbers that far exceeded what is typically seen in orthopedic research and still do to this day. People quickly realized I wasn't joking. I figured out how to work approximately 20 hours per week, generate seven figures plus in annual gross revenue, and maintain a business model with relatively low overhead. My clinic averages approximately $100,000 per month in revenue with about $40,000 per month in expenses. That allows me to take home roughly $60,000 per month while taking four weeks of vacation each year working only 20 hrs+ a week! But here's what makes this different. Just like I developed systems for diagnosing and treating patients, I developed systems for building a successful clinic. I don't just teach theory. Pt. Who Has A Simple Issue I Probably Cannot Fix, Here's Why...
I am working with a patient in her 30s who has had SI joint pain and Achilles tendinopathy for over a year. Clinically, this should be pretty straightforward for me. I would normally put her on my protocols and generally expect her to improve. The problem is she is not responding to either rehab program. Problem after problem after problem. This is unusual for me because I do this all day, every day, and I honestly can't remember the last time I saw this. Some patients improve and then have a flare-up in the middle of treatment, but to not feel any different at all is very strange for me. When I realign her SI joints, she has no pain. When I perform a posterior mobilization of her talocrural joint, her pain is immediately relieved. This confirms that these mechanical issues are contributing to her symptoms. Now I just need to make the changes "stick," if you will. I taught her several acupuncture points to stimulate with a TENS unit, which immediately relieved her low back pain. As soon as I showed her, her response was the same thing I've been hearing for weeks: "Yeah, but it's so bad." I put an SI belt on her and it instantly relieved her pain. I told her to use it until we could strengthen her adductors to help control the outflare. She replied: "Yeah, but it's so bad." No matter what solution I provide, I hear the same response. I've had patients who couldn't perform the Mulligan bike-tube mobilization for posterior capsule tightness because the tube irritated a tender area. In those cases, I simply reposition the tube and the problem is solved. I find a workaround. With this patient, she says she simply cannot do it. I've honestly never seen anything quite like this one. I'm starting to suspect that she may be subconsciously sabotaging the rehab process. It resembles classic pain catastrophizing: you provide a solution, and the patient immediately focuses on why it won't work without ever really trying it. These patients are not as common as I think many providers believe, but when I encounter one, my heart sinks.

Financial gain or emotional gain. Some people "need" to remain sick because they get an emotional benefit from it maybe from a spouse or child or family member. But then again, they paid you your fee so I would think they desire to be better. I had a patient once that did something similar. After a few times of that, I stopped and asked her point blank, "do you want to get better or are you wasting your own time trying to find another thing that doesn't work? If so, let me get you a refund so you can do something else." She straightened up and magically got better after a few more weeks. I have written testimonials and everything lol.

I treat what I call the "I have failed everything patient." I tell people once you start learning how to properly diagnose these conditions and have protocols to treat them effectively, you will have a level of job satisfaction that you never thought possible. People ask me "Ryan, don't you want to work with athletes?" No, I don't I have worked in the NFL and all kinds of pros with my methods being used in pro sports and I would rather work with patients like this. This patient told me "I am in so much pain for so long and no one takes its seriously" she was sobbing, (very typical people cry in my clinic when someone finally understands what was wrong and get them out of pain!)
