In my institution, audit trails are often weak due to manual systems and lack of regular review. Automatic and regularly checked audit trails improve transparency and compliance.

In my institution, records are still mainly manual and are easily damaged or lost. Although a spreadsheet is used to track files, it does not capture the information inside the files. This means there is no proper version control or single source of truth, and if a file is lost, the information is lost as well. This increases the risk of errors, poor service delivery, and non-compliance.
Managing access to records
Access controls protect records from unauthorised use while supporting legitimate business needs. Over-permissive access increases risk, while overly restrictive access hinders productivity. Balance is essential. Questions: 1. Are access rights reviewed regularly? 2. Are they role-based? Action: Check access permissions for one shared repository.
