Hi everyone, For tonight’s session, we’re going to talk about something that every nurse needs to understand if they want to partner with primary care practices. Many nurses approach providers by talking about tasks. “I can help with follow-up.” “I can check in on patients.” But providers aren’t looking for tasks. They are looking for solutions to the structural challenges inside their practices. The five biggest challenges I consistently see in primary care are: • Time constraints• Documentation burden• Follow-up gaps• Revenue leakage• Staff burnout In the slides I’m sharing here, I walk through each of these challenges and explain how nurse-led care management can help address them. This is the foundation for learning how to convey your value to providers. If you can clearly explain how your services help solve these problems, the conversation with providers changes completely. Take a few minutes to review the slides before our session tonight. We’ll walk through them together and talk about how to use this framework when positioning yourself to primary care practices. What are some questions Nurses have about starting a Business in CCM
What are the big questions you have about CCM and starting a business, what are your challenges. I hated marketing my services, I believe I would have done so much more if I were more active with my marketing. But I'm grateful for the opportunities I have now as they are aligned with how I want to show up and what I love doing. Let's talk about it tonight during our weekly live event. Hope you can join us.

Thank you for this PDF. As it relates to your opening question, "What are the big questions you have about CCM and starting a business, what are your challenges," mine seem huge. I have established the framework and complete program for my care management company. It's maternal health based & the OB offices I've spoken with are interested in learning more. I have been in talks with a software management/RPM company that can integrate with almost all of the EMR systems and it's developed & owned & operated by a Physician. The OB offices that want to learn/hear more see the value in my care management company being the "staff", (although independent of their offices), that works with patients placed in care management. Getting offices to invest in the software management/RPM company is going to be the challenge. It's difficult for me to see the ROI as it relates to Medicaid/commercial (some) insurance reimbursement. How can I market the complete system, (my care management company & software management/RPM company), in this fragile economy?

The 2026 RPM code set (what to bill now) A) Setup / patient education (unchanged) - 99453 – Initial setup + patient education on use of equipment (still your “start the program” code). B) Device supply + data transmission (this is where the big change happened) - 99445 (NEW for 2026) – RPM device supply with daily recordings/alerts and transmission for 2–15 days in a 30-day period. - 99454 (descriptor clarified for 2026) – RPM device supply with daily recordings/alerts and transmission for 16–30 days in a 30-day period. C) Treatment management (time + interactive communication) - 99457 – RPM treatment management, first 20 minutes in the calendar month requiring interactive communication. (No code change noted for 2026.) - 99458 – Each additional 20 minutes (add-on to 99457). (No change noted for 2026.) - 99470 (NEW for 2026) – RPM treatment management, first 10 minutes in the calendar month requiring 1 real-time interactive communication with the patient/caregiver.
