Answer is A. When the human immunodeficiency virus (HIV) infection status of any client is unknown, which includes a newborn with an HIV-positive mother, standard precautions are implemented. Standard precautions were developed by the U.S. Centers for Disease Control and Prevention in an effort to reduce the risk of transmitting bloodborne and body fl uid pathogens when the infectious status of a person is unknown. Generally, if the HIV-positive status of the mother is known prenatally, antiretroviral therapy is begun before delivery to significantly reduce mother-to-neonate transmission.

Appendicitis is characterized by pain and tenderness in the lower right quadrant, midway between the umbilicus and the crest of the ilium. Test Taking Strategy— Analyze to determine what information the question asks for, which is the anatomic location of the tenderness experienced by a client with appendicitis. Recall that the classic sign of appendicitis is pain when pressure is applied and then released over what is known as McBurney’s point, in the lower right quadrant. The phenomenon is known as rebound tenderness. Review the signs of appendicitis if you had difficulty answering this question.
