Quiz generator app?

Hi Everyone, can you help?

I am trying to create an assessment style quiz, as a lead magnet, to collect customer email address for the result. For example if you take a learning style's quiz you will finally discover which type of learner you are, out of a possible 3, Visual, Auditory or kinaesthetic, after answering a series of questions about the way you learn.

Can you recommend an app or website that I can build this type or quiz/ survey on please?
