Hereβs something practical for you all: a simple Cardiology OSCE History Template you can use in exams or placements. Some of you may know it but some of you may find this helpful.
Structure:
1οΈβ£ Presenting complaint
2οΈβ£ History of presenting complaint (onset, character, radiation, associated symptoms, timing, severity)
3οΈβ£ Past medical history (cardiac + general)
4οΈβ£ Medications (cardiac, anticoagulants, risk factors)
5οΈβ£ Family history (sudden death, cardiomyopathy, IHD)
6οΈβ£ Social history (smoking, alcohol, occupation, exercise tolerance)
7οΈβ£ Systems review (chest pain, palpitations, dyspnoea, syncope, oedema)
8οΈβ£ Summary + working diagnosis
