Structure:
1️⃣ Presenting complaint
2️⃣ History of presenting complaint (onset, character, radiation, associated symptoms, timing, severity)
3️⃣ Past medical history (cardiac + general)
4️⃣ Medications (cardiac, anticoagulants, risk factors)
5️⃣ Family history (sudden death, cardiomyopathy, IHD)
6️⃣ Social history (smoking, alcohol, occupation, exercise tolerance)
7️⃣ Systems review (chest pain, palpitations, dyspnoea, syncope, oedema)
8️⃣ Summary + working diagnosis
