NOTICE OF DISPUTE AND REQUEST FOR SUBSTANTIATION

Notice No. 99999-66666 – Response and Request for Substantiation
Dear Sir/Madam,
I write regarding the alleged charge referenced above.
For the avoidance of doubt, I do not admit liability for the alleged charge. Before any enforcement,
collection activity, adverse reporting, or further processing of my personal data continues, I require
full lawful substantiation of the claim.
Accordingly, please provide the following within 7 (seven) days of the date of this notice:
1. The statutory, contractual, or regulatory basis relied upon for the alleged charge.
2. A complete breakdown showing precisely how the amount claimed has been calculated.
3. Copies of all notices relied upon, together with evidence of lawful service.
4. The evidence identifying me as the person legally liable.
5. The evidence of any alleged loss, damage, breach, offence, or other enforceable obligation
relied upon.
6. The identity and legal capacity of the claimant, creditor, authority, or party asserting
entitlement to payment
7. Confirmation whether the matter is pursued as a civil debt, contractual claim, statutory
charge, administrative penalty, or criminal matter.
8. Copies of all contemporaneous records, account notes, system records, audit logs,
correspondence, and any other documents relied upon in support of the alleged liability.
Where it is alleged that a loss, damage, injury, or offence has occurred, please identify the injured
party or legal person said to have suffered that loss together with the evidence relied upon.
Pending receipt of the above information, the alleged charge remains formally disputed. Accordingly,
you are requested to suspend all enforcement action, collection activity, third-party referral, doorstep
contact, and adverse reporting until this dispute has been properly resolved.
For the avoidance of doubt, I remain willing to resolve any lawfully established liability. Upon receipt
of clear, lawful, and sufficient substantiation establishing the legal basis of the claim, I will give the
matter my immediate attention and, where liability is properly established, will discharge any sum
lawfully due.
However, should you fail to provide the clarification and substantiation requested within 7 (seven)
days, and should you fail to dispel the matters in dispute with lawful evidence, I shall regard your
failure as an inability to substantiate the alleged claim.
If, after a further 14 (fourteen) days, no lawful clarification or substantiation has been provided, I
shall consider the alleged claim to have been abandoned and the dispute concluded unless and until
competent evidence establishing liability is produced.
Nothing contained within this correspondence shall be construed as an admission of liability,
acceptance of the alleged charge, waiver of any rights, or consent to any enforcement action.
All future communication must be in writing by email only.
