Need advice — clinic client came back after pricing, scope is heavy
Hey everyone 👋
I need some honest advice from people running agencies.
Context:
I spoke with a medical clinic on Dec 23. At the time, they were talking to multiple agencies.
Today, they called me directly and decided to move forward with me — this wasn’t fully planned.
We discussed $2,000 CAD setup + $1,000 CAD/month, but since then, the real scope became much clearer and it’s way more complex than a normal setup:
  • AI voice receptionist
  • French + English
  • 2 clinic locations
  • 3–4 appointment calendars / types
  • Multiple clinicians
  • Some patients choose their clinician
  • Doctor rotates between clinics (logic like “which clinic is he in today”)
  • Needs direct API integration with the client’s CRM
  • ~1,500–2,000 minutes/month
I haven’t collected payment yet and haven’t done the onboarding call yet.
The issue is:
  • I can’t realistically build this alone
  • I’m looking at Upwork for help, but advanced builds like this often cost $3k–$6k+
  • I might get lucky and find someone around $2k–$3k, but that still puts pressure on margins given the price already discussed
I’m not trying to bait-and-switch — just trying to handle this professionally and sustainably.
Questions:
  • How would you personally handle this situation?
  • Would you reframe pricing before onboarding?
  • Any advice from people who’ve dealt with clinics or heavy custom builds?
Also, if someone here is strong with APIs / voice agents / complex workflows and open to paid help or co-building, I’m open to that.
