I can see how tedious this can be both for you and the patient. In our hospital, we are actually working on transitioning to the EPIC system that is meant to solve this issue - at least between our clinics and hospital and specialty healthcare systems that we partner with. However, there are still a number of other healthcare clinics and hospitals that are not using this system and it is only as good if EVERYONE is on the same platform. There is a lot of opportunity for improvement, especially on the national side.
