Interactive Case Study
Quick Case 🩺 A 34-year-old female presents with 4 days of persistent nausea and vomiting. She’s having trouble keeping food and fluids down and is feeling increasingly weak. No diagnostic testing has been completed yet. Before jumping to labs, imaging, or a diagnosis, think through the presentation. What’s on your differential? What are the top 3 conditions that could actually explain why she is having nausea and vomiting? What else do you want to know? Think about the history questions that could completely change your differential. Pregnancy possibility? LMP? Abdominal pain? Diarrhea? Fever? Urinary symptoms? Medications? Cannabis use? Sick contacts? Recent food or travel? Previous abdominal surgery? Headache or neurologic symptoms? And don’t forget the “what could I absolutely not miss?” diagnoses. The differential isn’t a list of everything that might be abnormal. It’s an organized supportive list that are the most likely causes of your chief complaint 💡 Think chief complaint → differential → history & physical →refine differentials→ diagnostics Your diagnostic testing should help you rule in or rule out the diagnoses you’re already considering, not create your differential for you. @Nikkie Miller I was thinking of the things you mentioned in the question: when was her LMP, had she eaten anything usual prior to the symptoms, does she have a history of other GI issues heartburn, ulcers. The canabis use I had not thought of but that makes sense.

Hello ! My name is Ashley. I’m in the DFW area of Texas. I am at the very beginning of and start class end of September at UTA. I’ve been a nurse since 2007 and have done many different specialties. After needing a break from the hospital scene, I’ve been working as a nurse educator and program manager for a maternal child program for the last 3 years. I am completely nervous about going back to school. The BSN track was not kind and I vowed never to return…but alas, I’m here. I’m on the FNP track, although primary care not my end goal. Hoping that something will spark my interest. Glad to have been invited here. Glad to have been invited here to have discussions. The online format gives me some pause because of course the last time I was in school you had to been in person. 🫣🥸
