Hi Frankie, I work in medical imaging. For me personally I have always been very open about mistakes, as nobody is infalable, we all make mistakes. Mistakes are learning opportunitues for improvement. I found discussion with line managers or collegues to be very helpful in processing what has happened and what can be learnt or done better. But essenrtially my process: 1. Recognise & admit, 2. Communicate (to line manager, patient if involved etc), 3. Root cause analysis, 4. Rectify/improve/learn. I make every effort to ensure I am not being careless or negilent in my clinical work. So from a mental side when a mistake has happened I know its not a reflection of me as a person but of a broken process or system.
