Has Anyone Actually Asked for These Tests?
After Tuesday's post, I want to hear from the community: have you ever asked your doctor for fasting insulin or HOMA-IR? What happened? Were they receptive? Did you get pushback? Did it lead anywhere? Your experience matters, both as validation for others in the same position and as real-world data about what we are actually up against. There is no wrong answer here. I have asked a patient’s PCP to order some of the tests mentioned here and their response was basically no, we will run what I want to run. So now I will have to write a letter directly to the PCP to see if I can get him to order the labs since that will allow her to use her insurance. In the meantime I’m still waiting to get my ability to order labs directly set up with Quest Diagnostics. I’m hoping this reduces the need for PCPs to order labs.
What Came Before the Symptoms?
I want to ask the community something: looking back, what was life like 3 to 5 years before your symptoms showed up? Were you working extreme hours? Going through a difficult period? Under-eating to manage your weight? Not sleeping? Perimenopause beginning? Often the pattern makes sense in retrospect. The body was adapting, protecting itself the only way it knew how. Interesting. I like this perspective. My symptoms started in 2021. In 2016-2017 I purchased a business I had been an employee out. It was a very stressful experience. Prior to that, all I can remember is being pretty active, working, and playing. I would sometimes workout 2x/day, before work and after work. I participated in outdoor sports on the weekend. No big complaints. Suddenly in 2021 I started to feel very unlike myself. From 2021 until now I’ve been trying to figure out what was going on and find solutions. I thought it was my IUD, so I had that removed. Nothing really changed. I started HRT in November 2025 and the severe insomnia I was having (which was not so slowly wrecking my life) almost completely went away. My anxiety. Since then, I’ve been on a journey to figure out all the other symptoms since then. I’m still noticing brain fog / difficulty findings words when speaking, low libido, weight gain and having difficulty loosing the weight.
Lab Specifics
I’m curious what you recommend if someone’s TSH is typically in the 2 range but total T4/free T4 and free T3 are usually within range? Vit D is lowish in the 40s, ferritin good.

Thyroid 40%, insulin resistance 30%, cortisol bal 20%, sex hormone bal 10%, gut health 0%. Although I will say, I don’t feel this is accurate for me. There were a few questions that didn’t have an answer for me. For some, I had to choose answers based on symptoms I used to have but no longer have since starting HRT.
