@Andy Openshaw This sounds good, definitely beyond the 80% rule. I think you have the right approach on working on the transitions. I sometimes analyze the change by finger - so which finger is traveling the farthest, what finger stays in place, is there an easy way to pivot - just go slow enough to understand it at a deeper level - instead of throwing my fingers in the air and trying to come down in the right place. I think the counting thing just takes practice and time - and most struggle with it in the beginning. I agree with you that when I play with the metronome or drums and bass that I can depend on them to hold the downbeat and keep me centered - but there are issues with that. In the band situation, it's everyone's job to keep time - which allows the bass and drums to play with more freedom - and sometimes you are playing alone. If you've watched my comping video's, you can see that I try to break things into blocks - like a Charleston starting on the 1 or the 3 like you have in 2.1 and 2.2 - and work on counting that specific thing (out loud or in my head) - so when it comes to play the part the counting comes naturally. Second, you initially need to practice counting slowly. When I have a difficult section to count - I'll work on it as slow as it takes so I can get hear the timing. You have a good feel for the rhythms so far, it will come with your continued effort!
