• Gender: Male

• Age: 41 years old

• Ethnicity: African-American

• Height/Weight: 5’7”, 145 lbs

• Diagnosis:

• Intellectual & Developmental Disability (IDD)

• Autism Spectrum Disorder

• Bipolar Disorder

• Hypertension

• Cognitive & Developmental Delay

• History of Aggressive & Agitated Behavior (requires daily medication)

🔹 Behavior & Functionality:

• First-time group home resident – has never been in an AFC setting

• Recently hospitalized following aggressive behavior toward his mother and family

• Exhibits agitation and aggression when off his medications

• Ambulatory – can walk and manage basic mobility

• No history of smoking, drinking, or substance abuse

• Displays cognitive delays and will need close behavioral support

🔹 Housing Needs:

• Private room strongly preferred

• Licensed IDD providers only – must be equipped to support IDD and behavioral health clients

• Medication management is a must (client must remain med-compliant)

• Monthly income: $1,500–$2,000

• Licensed providers are expected to bill Medicaid/insurance for IDD-related services

• Client qualifies for additional support services due to IDD diagnosis

• Client is currently in a Texas hospital and is ready for discharge

• Immediate placement needed upon identification of appropriate provider
