• Primary Diagnosis: Chronic Macrocytic Anemia
• Additional Conditions:
• PAFib (not on AC due to prior bleed)
• Hypertension
• CKD Stage 3 (without deficits)
• Stroke (2019)
• Chronic physical debility
• Chronic protein-calorie malnutrition (requires nutritious meals to regain strength)
• Dementia (progressing; requires redirection and reassurance)
