Lead Report creation guidence

Hi team,

Need guidance on setting filters for report creation.

Case - We have a user who only generates lead from various sources.

We want to know -

1. How many leads he generated, and what are those?

2. What are the company names we got from there? (Some leads are converted, and some are not)

3. How many opportunities we have created from those leads, and what are those?

All these information we need in a single report.

Please guide.
