Priority Assessment-When A Patient Seems "Off"
Changes in mental status such as new confusion, restlessness, agitation, or “not acting like themselves” can signal hypoxia, sepsis, or poor perfusion. Restless is often one of the very subtle manifestations that can indicate a change in the patient's condition. Even mild disorientation is significant and warrants further assessment, intervention and notifying the Physician/Provider.
