What Is LL-37?
LL-37 is the only member of the cathelicidin peptide family identified in humans.
It consists of 37 amino acids and is naturally produced by the body as part of our innate immune defense system — essentially one of the body's first lines of defense when something potentially harmful shows up.
But LL-37 is more interesting than simply being an antimicrobial peptide.
Research suggests it also plays roles in immune signaling, inflammation, biofilm activity, tissue repair and wound healing.

1. THE MEMBRANE DISRUPTOR
LL-37 can interact directly with microbial membranes.
Because of its structure and positive charge, it can bind to certain negatively charged microbial surfaces, destabilizing their membranes and contributing to the destruction of the microorganism.
Research has demonstrated activity involving bacteria, fungi and some viruses, although its effectiveness varies considerably depending on the organism and biological environment.
2. THE BIOFILM DISRUPTOR
Some microorganisms don't just float around individually.
They can organize themselves into biofilms — protective communities surrounded by a matrix that helps shield them from the immune system and can make them much harder to eliminate.
LL-37 has been heavily researched for its ability to interfere with biofilm formation and disrupt biofilm behavior in certain experimental models.
This is one of the reasons LL-37 gets so much attention in antimicrobial research.
3. THE TISSUE REPAIR CREW
This is where LL-37 gets especially interesting.
Its role doesn't stop with microbes.
Research has associated LL-37 with:
Cell migration • Epithelial repair • Angiogenesis • Wound healing • Tissue remodeling
In other words, LL-37 appears to participate not only in defending damaged tissue but also in helping coordinate some of the processes involved in repairing it.
🧠 BUT THERE'S ANOTHER SIDE TO LL-37
LL-37 is sometimes casually described as an “immune booster.” A better description is immune modulator.
LL-37 can influence immune-cell recruitment, inflammatory signaling and cytokine activity. Depending on the biological environment, those effects may be helpful — or potentially problematic.
More immune activity does not automatically mean better immune function.
And while LL-37 has generated considerable laboratory and early clinical interest, the claims sometimes made about systemic LL-37 use in peptide circles go considerably beyond the human clinical evidence currently available.
