SS-31 comprehensive guide
SS-31 (Elamipretide) for Beginners
Peptide Beginners Academy Educational Guide 🧬
SS-31 (also called elamipretide) is a small, man-made peptide that targets the mitochondria — the “power plants” of your cells. It is one of the more studied mitochondria-focused peptides and has real human clinical trial data behind it.
⚡ What Is SS-31?
SS-31 is a four-amino-acid peptide (D-Arg-Dmt-Lys-Phe-NH₂). It was designed to go inside mitochondria and interact with a special fat called cardiolipin that sits on the inner mitochondrial membrane.
In simple terms:
When mitochondria get stressed or damaged (from aging, injury, or certain diseases), cardiolipin gets disrupted. This leads to less energy (ATP), more harmful free radicals (ROS), and poorer cell function. SS-31 helps stabilize cardiolipin so mitochondria can work more efficiently again.
🔋 How Does It Work in the Body?
It concentrates heavily inside mitochondria.
It binds to cardiolipin and helps keep the inner membrane structured properly.
This improves the electron transport chain (the process that makes most of your cellular energy).
Result: better ATP production and lower excess oxidative stress, especially in mitochondria that are already underperforming.
Tissues that use a lot of energy — heart, skeletal muscle, kidneys, and eyes (retina) — are of particular interest because they are packed with mitochondria.
📊 What Do the Clinical Trials Show?
Barth Syndrome (a rare genetic condition that damages cardiolipin)
This is the only FDA-approved use so far. In September 2025, elamipretide (brand name Forzinity) received accelerated approval to improve muscle strength in patients with Barth syndrome who weigh at least 30 kg. Open-label data showed meaningful gains in walking distance and strength.
Mitochondrial Myopathy & Heart Failure
Early studies had some positive signals on walking ability or heart measures. Larger Phase 3 trials mostly did not meet their main goals. Results have been mixed.
Aging Muscle
One controlled study in older adults found that a single intravenous dose improved the muscle’s ability to produce ATP compared with placebo. This showed that age-related drops in mitochondrial energy production can sometimes be improved.
⚠️ Safety Overview
Across many trials (hundreds of patient-years of exposure), SS-31 has generally been well tolerated. The most common side effects are mild-to-moderate injection-site reactions (redness, itching, pain, or bruising). Serious systemic problems have been uncommon.
👁️ Eyesight Findings (Dry AMD / Geographic Atrophy)
The retina has a very high density of mitochondria, so researchers have tested SS-31 in age-related macular degeneration (AMD).
ReCLAIM (Phase 1, open-label): 40 mg daily subcutaneous for 24 weeks. People showed average improvements in standard vision (about +3.6 to +4.6 letters) and especially in low-light vision (+5.4 to +5.6 letters). Reading ability in dim light also improved.
ReCLAIM-2 (Phase 2, placebo-controlled): The main goals (low-light vision change and growth of geographic atrophy) were not met. However, SS-31 slowed the loss of the ellipsoid zone (a key layer of photoreceptors) by roughly 43–47% compared with placebo. More people on the drug also gained 10 or more letters of low-light vision.
A larger Phase 3 trial (ReNEW) is still ongoing. The structural protection of photoreceptors and low-light vision signals are the most discussed findings so far.
💉 Example Research Protocol (Educational Only)
Clinical trials in disease states usually used 40 mg subcutaneously once daily.
In research and community settings exploring general mitochondrial support, lower doses of 5–10 mg once daily are commonly discussed.
Sample beginner-friendly research schedule:
Weeks 1–2: 5 mg once daily (to check tolerance)
Weeks 3–12: 10 mg once daily if well tolerated
Timing: Same time each day (morning is popular)
How long: 8–12 weeks, then reassess or take a break
Injection: Subcutaneous (belly, thigh, or upper arm). Rotate sites.
Optional schedule: 5 days on, 2 days off
Reconstitution tip (typical 10 mg vial):
Add 1–2 mL bacteriostatic water. Store the mixed solution in the refrigerator (2–8 °C) and use within about 28 days. Keep the dry powder cold or frozen for longer storage. Protect from light.
💡 Extra Tips for Beginners
SS-31 is sometimes discussed alongside other mitochondrial supports (MOTS-c, NAD⁺ precursors, CoQ10, good sleep, and exercise). These address different parts of mitochondrial health.
Track how you feel (energy, recovery, exercise tolerance). Some people also note changes in low-light vision.
Quality matters — look for pure research material with proper testing (COA).
Injection-site reactions are the main practical issue for most people. Proper technique and site rotation help.
